The six-year collaboration agreement focuses on delivering digital care solutions for the treatment of chronic conditions with potential initial priorities such as heart failure and chronic obstructive pulmonary disease (COPD), as well as a clinical insight platform that enables Highmark Health to use data to guide patients through personalized pathways for care respectively. These solutions will be part of Highmark Health’s Living Health model for patients and clinicians.

These solutions are leveraged using Verily’s advanced analytics and artificial intelligence and Highmark Health’s clinical knowledge and expertise, gained through its unique relationship and experience with patients as an insurer and provider. You’ll be refined through a robust, nimble testing and learning environment, evidence and clinical validation for condition-specific solutions.
